Please bring following items to the initial visit;
- Medical insurance card
- Form of identification such as a driver’s license
- Medical records relevant to the visit including
- Lab results (copy of blood work within 30 days)
- Procedural or operative reports (copy of previous operative report)
- Radiology reports
- Radiology films (CT, PET or X rays) in a CD format if the films are performed at a non-Methodist affiliated hospital relevant to the medical condition. Please contact the hospital where the film was done for a copy of the films on a CD.
