Please bring following items to the initial visit;
Medical insurance card
Form of identification such as a driver’s license
Medical records relevant to the visit including
Lab results (copy of blood work within 30 days)
Procedural or operative reports (copy of previous operative report)
Radiology reports
Radiology films (CT, PET or X rays) in a CD format if the films are performed at a non-Methodist affiliated hospital relevant to the medical condition. Please contact the hospital where the film was done for a copy of the films on a CD.
